How late can I be for my appointment?
You have to be here at your appointment time or before. Late arrivals may be rescheduled for a different day, and the appointment marked as a missed appointment.
How do I request my records from my previous doctor?
In order to get your records from your previous doctor, complete a Medical Records Release Form. You will find a copy on the Forms page. Please complete the form and return it to the clinic. Forms can be returned via email (firstname.lastname@example.org) or faxed to (903) 677-4234. Please call our records department with any questions at (903) 675-7376 ext 201.
What do I need to bring to my appointment?
Please bring your insurance card and identification.
What types of insurance do you take?
We take most commercial insurances, Medicaid and Medicare. Please call your insurance carrier to make sure that we are on your plan.
How do I become a patient?
It's very easy to become a patient. Just call one of our friendly staff at 903-675-7376 and they will be more than happy to make you an appointment.
